One of my favorite C++ books, this describes most of the common pitfalls of C++ programming. It goes in depth and explains not only what not to do, but also why not to do it, and usually offers good alternate methods. Despite not being a reference book per se, it offers good insight into many of the technical details behind the scenes in a C++ program's operation.
