Can anyone recommend some books on modern assembly programming (with Visual C++) and using it to optimize games?

It's not a book, but the low level curriculum series of blog posts on AltDevBlogADay is pretty good. It's kind of an intro to being able to read assembly, by looking at examples of asm that's generated by MSVC from C++ code.
