Hmm, I filled out the form, hit submit and got:

Oops, something went wrong.
Something bad happened. Don't worry, though. The Spreadsheets Team has been notified and we'll get right on it.

Anyway, I think you should use checkboxes instead of radio buttons for questions where it makes sense, in quite a few of them I wanted to select more than one option Posted Image

(although I don't know if google docs lets you do that).

Something went wrong with mine as well, as with the previous poster. However, it was quite goid. But, you should add an 'other' field if possible, as i found myself wanting to answer differently for some questions
