There are several different ways, depending on the type of hook being used. An enormous amount of Windows functions allow programs to hook into them or intercept them or trace them or replace them, and it can happen at any level from drivers to debugging tools to running all of the OS inside a debugger-based virtual machine.

Many methods are completely undetectable by your application.

Wow, that word has completely lost any semblance of real meaning, hasn't it? :/
