The MSDN documentation on DDS files states that "For simple uncompressed textures, the file should contain DDSD_PITCH and a DDPF_RGB pixel format surface." However, when I make an uncompressed DDS texture using the DX Texture Tool, the resultant DDS file does not have DDSD_PITCH set in the dwFlags; and, in fact, the pitch value in the struct is 0. What gives?

Well, since the docs and the tools contradict, maybe the DX Texture tool isn''t exporting correctly. Then again, maybe the docs are wrong. That would be the simple explanation
