Designing a Game World

How do I go about designing a world Should I make a Model that is the whole world then load that or make each part of the world then place the parts into the game to generate the world?

This heavily depends on the type of world...

For an indoor game, consider partitioning your level into polygons and building a culling system to select which to render.

For an outdoor game, having the ability to place objects on a heightmap can prove useful.

For a game that is played in space, placing objects around the map is cool.

For a tile based game, definitely use tiles (duh).
