I liked Assembly Language for Intel-Based Computers. It covers both 16-bit and 32-bit ASM and goes into advanced DOS stuff as well as making Windows GUI programs in ASM. The CD that comes with it has a useful libary with input/output routines (as well as other stuff) and it comes with the MASM assembler.

Asm is specific to the processor, you should go to Intels homepage or google "x86 instruction set"
