proper way to credit code's author?

Hello, I've just finished writting a demo that I plan on sending to different game dev companies. My demo consists of my personal code as well as code written by other individules. I want to know the appropriate manner in crediting these people. Should I include a readme file listing their names? Should I comment out their name in their code? Thanks!

First of all you should consult the original authors and see if they have a problem with you using their code in a venture that might result in profit for you. After you've got their permission, you should credit them both in the documentation (either in the appendices or on the front page) AND in the source. Always provide information on how to contact the authors and links to their website (if any).
