Multi - Direct3D adapters

I've just added a a second graphics card to my system. I now have a TNT2 as the primary adapter and a Geforce2MX as the secondary. My problem is that while my program identifies both adapters I can't get a Device to the 2nd adapter - Direct3d8::CreateDevice is failing. The window for my app is being created on the primary monitor and I'm wondering if that's the reason why? I did have it working for a short time, then I played with the 'Send application to monitor' options when it was running and since then it can only create devices on the primary monitor. A second issue is that if I have D3D running fullscreen on one monitor, as soon as I shift focus to an application on the other monitor (like the debugger) then my D3D app is minimised. Any way around this?
