I have been thinking about the best data storage for my current project and I've been going back and forth between some incarnation of SQL or a flatfile database. Do most MMO games tend to use SQL?

I would go with SQL.
You can pull mySQL right out of the box and jam it into your game code - they have the speed down. The organization is good and the extensibility lends to properties of what could be thought of a scripts ( for item properties n such ). 2c.

Another vote for SQL (although RDBMS would be the appropiate acronym [grin]).
