How to do underline with ID3DXFont?

How do I make a font that is underlined with ID3DXFont? Or is it done in the DrawText function? If so how do you make underlined text?

You'll need to use D3DXCreateFontIndirect with the LOGFONT::lfUnderline field to true. Of course, that means you need an ID3DXFont for every variation of your font (i.e. one for bold underlined, one for underlined, one for italic, ...etc)
