making a texture out of an image buffer

How do I create a texture from an image buffer when all I have is the buffer, width, height and format? For example if I have a 256x256 24-bit RGB image what do I do?

Another way:

1) Create the texture
2) Get level 0 of the texture (IDirect3DTexture9::GetSurfaceLevel())
3) Use D3DXLoadSurfaceFromMemory() to load into that surface
4) Remember to release the surface
