You could just interpolate between the FogColor and the usual diffuse Color with the Depth as the blending function...

I don't know what you mean by doing smoke with shaders?! usually u use particlesystems with substractive blending

Edit:you could do smoke with Fluidsystems on the GPU but i have no tutorials for that. I think there's a shader for Fluids at nvidia.com but i don't know.
