How to learn Pixel and Vertex Shader Programming

Hello, I was wondering what would be a good way to begin learning about Pixel and Vertex Shader Programming. I have been using Direct X for quite some time but have never grasped the programmable pipe lines. Looking at the HLSL examples provided in the SDK show lots of information but dont realy explain what each line in the sample does or how they came up with it. So I was hoping that someone could point me to a book or a web page somewhere that broke down the programmable piple lines in a step by step manner. Thanks
