It looks like a custom model format that the author has created. Looking at the source code, the file seems to be organized thus:
/* custom types */

// vector type
struct vector
float x;
float y;
float z;

// vertex type
struct vertex
vector normal;
vector position;

// polygon type
struct polygon
vertex vertices[3];

/* file definition */

// number of polygons in file
int num_polys;

// then the polygons
num_polys * polygon;
