Decent B-Tree implementation?

Hi, Can you please recommend me a decent, complete, B-Tree implementation? I've had a look at a few but am unsure of how full-flegded they are (some, for example, suffer for poor delete implementations). Is there a 'standard' B-Tree library that people use? Cheers, O.
