WTF DirectSound SetVolume and global focus

I've just discovered some really strange behaviour trying to set the volume of music in my game with IDirectSoundBuffer::SetVolume. I created my buffer with DSBCAPS_CTRLVOLUME and then used SetVolume(-600) to chop 6dB off the volume, but for some reason it had no effect on either of the machines I tried it on. Panning (with DSBCAPS_CTRLPAN set, obviously) worked as expected, but not setting volume. I checked the result from the function and it was returning DS_OK. Then I gave the buffer DSBCAPS_GLOBALFOCUS and the volume control began working. What the hell is going on?
