How to apply perspective to an image?

Hello, I have an image that I want to apply a rotation by using a matrix transformation. I've seen some examples over the net about how to do it over the x and the y axis, but nothing about the z axis. I would like to do it not by using libraries like DirectX or OpenGL. Thank you very much :D

Matrices can do that kind of stuff:

or do you mean something more like:
http://freespace.virgin.net/hugo.elias/graphics/x_persp.htm

I could be less vague with a little more information about the problem.
