Those terms are, for the most part amorpheous and their definitions will vary with context. In general, a game developer is any person or organization that builds games in some fashion. A game programmer is a person who writes code in a programming language with the goal of building a game, and a game designer is a person responsible for helping to shepherd the "vision" of what the game looks and plays like (balance, level design issues, et cetera).

But of course, those roles can vary widely.
