I was wondering how in games like Dark Age of Camelot and World of Warcraft (just to single out a few examples), they do not need loading screens to travel from one zone to the next. with the exception of instances and a few others travel is relatively smooth.

Do a search for "terrain paging" or "terrain streaming" - there have been several threads on the topic. A google search will also give some useful results.

They load in new content in the background as you move towards the next area. When teleporting or entering an instance, the data isn't preloaded and thus it must load the surrounding from scratch.

A simple approach is to have a grid where game has loaded the squares around the player which are necessary to properly display the game world. When the player then travels over the grid, new sections are loaded in the background while others are removed from memory.
