Motivation to play
+ Winning against people. I love Starcraft more than probably any other game, and it's because I can sit down for 30-60 minutes and hope to win against another person of similar skill.

Overall of the ones listed I like most of them if done well, but in most games they tend to be done badly.
I just wanted to mention that my list was in no way meant to be comprehensive. I only spent a few moments considering those, to serve as an example of the type of motivation I was getting at. But in any case, your list is far better, and nicely detailed.
Thank you. Although mine's nowhere near comprehensive either since I only listed ones I like, lol.
+ Explore the world (related to worldbuilding). Seriously, I'm as giddy as a kid when it comes to getting a new cool game that I know has a cool world to explore. The more I can interact with it, the more I get out of it. And if such interaction involves killing the enemies presented in that world, then all the better.
I'd up the plus on Survival for me. I think nothing else gives me that rush like a tight situation in games does (any situation, any genre).

+ Increasing some number. Whether it's a skill or some arbitrary, pointless level, I can't resist the desire to improve it. It's just the nature of the RPG that addicts me.
+ Exploration. I like seeing things I've never seen before, and just occasionally enjoying the atmosphere. New content in say, a patch is a good motivation for me to play.
+ Competition. Multiplayer is probably the single best way to get people to play a game. If you can play it with your friends, and compete, and if you can take part in tournaments or ladders, it's just pure fun.
I guess I'm least motivated by the story. I like to hear the story, sort of like in a movie, but I don't kill some mutant queen bug for lore reasons. I kill her because I am required to in order to progress.
If actions actually mattered, I might be more motivated by story. However, there are very few games that let your actions truly influence anything.

Hmm exploration, I guess that appeals to me to with the exception that I have absolutely no interest in exploring randomly generated landscapes, and if there aren't any set items to collect or puzzles or treasure chests to find I'm not going to be interested in exploring.
In the category of puzzles I only really mentioned adventure game style one, I should mention that I also like speedpuzzle minigames (except for memory ones because I suck at those lol), and I also like physical puzzles where you have to figure out how to jump and move crates and use the environment to get to some ledge or island (not so cool if you die every time you screw up). Puzzle bosses that are invulnerable unless you do something clever are also good (again it's best if they only do a token amount of damage so you don't die while you're standing there trying to figure out how to fight it).

Another one for me is..
+ Discovering culture. Just finding background information, to learn more about creatures, people, and factions of the game world. It depends on the game, of course. With many, I wouldn't even care to find additional info. But some games make their setting interesting enough to actually make me seek it out. It's even more interesting when the extra knowledge enhances your ability to play the game, such as understanding certain behavior traits of an aggressive creature, or learning the customs of a certain people to better interact with them.

For me I really like character progression and exploration on my own terms.
Much less so than competition and reflex action.
My favourite genre? You've guessed it RPGs.

I'm really quite tired of saving the world and rescuing people, although a good writer thinking outside the box could still use them in some way that I would like. For example I would like a time loop sort of game where events went badly the first time you lived through them and now it's your job to make sure things turn out differently.
The Legend of Zelda: Majora's Mask. Year 2000. *drool*
First of all my game practice is a little rusted.
I used to play a lot of games. There are a lot of games that are funny but that's all. There too little games I remember, because fun and frustration can be ephemeral, the reasons for them are the ones you probably remember.
Among others, the following things are very important to me.
You gotta give me someone or something to love.
But not just because the character does (Ex: family, town...).
They have to do something for you.
I'll never forget the Zero first aparition in Megaman X, first he saves you then he sacrifices himself. Well maybe I was just kid by then, and didn't really speaked english (I speak spanish), but the point is he did something for me that wasn't trivial for him.
I wanna feel bad when something bad happens to him, I wanna feel good when helping him.
I can save the world, but If you don't make me care for that world then I won't feel the relief of doing it because I don't care.
You gotta give me someone to hate.
Not just a villain.
He have to do something treacherous affecting you or someone you care.
I wasn't really that attached to Sarah Kerrigan in Star Craft, but I hated the betrayel from Arcturus Mengsk. I was trying to save her, for the mission only, but anyways he betrayed ME, after all I was playing Kerrigan.
I wanna shout bad words at him. I wanna see him dead, but not for his treasure, not for saving the word, not even for him to stop annoying me, just because I hate him.
