Switching Video Modes in D3D

I am having problems with my D3D8 app. It default starts in HAL ( windowed ). When I try to switch to a full screen mode I get the message " A D3D Object has a non-zero reference count( meaning things were not properly cleaned up ). This sample will now exit. " I have looked through my code a bunch of times and can''t find any objects that were not released or deleted. Has anyone else come across this problem? If so, how did you get past it?
