From what I have seen, very very rare, if ever.
The way it normally works, is it is drawn by hand and then a person redoes the drawing in a graphics app. Then it is put into the game.

I think I've seen one flash game where the art looked like it was drawn on paper and scanned in. I haven't tried this sort of thing but I think the most difficult part (aside from getting the scan quality right) would be getting any animation frames to line up properly.
