what is a +3 reputation event with no "item" listed?

upvotes list an item, but i also get +3 events with no item.

someone followed topic?    but i think i've seen those as an item in the past, perhaps a +1....

The question really is: "Why is that the case and when will it be fixed?" OK, two questions...

[EDIT]

Edited for clarity.

Sort of fixed.. there is a title at least but it doesn't link right yet.

Getting better.

Article comments seem to be somewhat linked as well. Some are still blank (maybe journal comments). And the title displays my name, not the name of the user who rated me up/down?

Oh I suppose that would be better on the receiving end actually rather than just knowing a comment got upvoted.
