Outdoor portals.

In a game such as Everquest... worlds are huge. I wanted to know how they only rendered segments, or space within a certain area. I have stumbled upon the portals paper over at flip code but he mentions theres no really good way to use portals outdoors. ( Explaining that you would use them between doors and windows and such. Is it possible that they just make a fog layer ''x'' radial distance from the viewer? Thanks. Andy
