Thanks. I''m still unclear on one thing though. Do I have to release resources created with the D3DPOOL_MANAGED flag?

Yes anything that was created by DirectGraphics needs to be released and reloaded.

This includes:

All textures
D3DX interfaces
X Objects
Vertex buffers
Surfaces
