Does Sphere map just need one texture which is generated by taking photo for a reflective sphere positioned at the center of the scene?
If so, the scene behinds the reflective sphere can't be saw in the texture.

it's correct, it's used for cases when you don't need the scene behind the sphere, e.g. image based lighting.
