Purtroppo non sono un dx9 guru, se il tuo progetto fosse stato fatto in opengl mi ci sarei cimentato.

Try out...

It's possible that the error is connected to quad declaration, Vertex Declaration, FVF or similar...

Take a look and try to FIND THE BUG...
